These terms apply to services provided by Katie Bianconi LLC (dba Katie B's Pup Pad). They mirror the Pet Care Agreement you sign at intake, published here in full so you can always come back and re-read what you agreed to.
1. Your Acknowledgment of Risk
Dogs in daycare, boarding, walking, transport, and group play are exposed to risks that can't be fully eliminated, even with reasonable care.
I voluntarily accept all risks associated with my dog's participation in these services, including:
- Minor injuries such as scratches, cuts, punctures, strained muscles, missing hair, bruising, or abrasions
- Illnesses and communicable conditions
- Exposure to parasites, bacteria, and viruses
- Stress-related conditions
- Injuries from rough play or interactions with other dogs
- Injuries from slips, falls, equipment, fences, toys, or natural play
I understand dogs use their mouths, paws, and bodies during normal canine play and interaction.
I acknowledge that no daycare, boarding facility, or pet care provider can guarantee a completely injury-free or illness-free environment when dogs interact socially or are cared for in shared environments.
2. Health & Vaccination Requirements
Florida law and our policies require current vaccinations on every dog in our care.
I certify that my dog:
- Is current on all required vaccinations (Rabies and DHPPT minimum; Bordetella strongly recommended)
- Is free of known contagious illness to the best of my knowledge
- Has not shown vomiting, diarrhea, excessive coughing, sneezing, or contagious skin conditions in the past 48 hours
- Is in suitable health to participate in daycare, boarding, walking, or transportation
I acknowledge that despite vaccination requirements and reasonable safety protocols, dogs in social or boarding environments may contract Bordetella/kennel cough, canine respiratory illnesses, Giardia, Coccidia, roundworms, hookworms, tapeworms, or other contagious viruses, bacteria, or parasites.
I understand vaccines are not guaranteed to prevent illness and that exposure can occur at daycare, boarding, parks, vet offices, public areas, or other social settings — and that parasites and illnesses may spread through airborne exposure, contaminated surfaces, feces, standing water, or normal dog-to-dog interaction.
I agree to notify Katie Bianconi LLC immediately if my dog develops a contagious illness shortly after attending daycare, boarding, or related services. Dogs without current Rabies and DHPPT records on file may be refused service at the time of drop-off.
3. Veterinary Care Authorization
If my dog needs medical attention while in your care, you have my permission to act.
If my dog becomes ill, injured, or needs emergency medical attention while under the care of Katie Bianconi LLC, I authorize staff to seek veterinary treatment as deemed reasonably necessary.
- Every reasonable effort will be made to contact me first
- I am financially responsible for all veterinary expenses incurred on behalf of my dog
- Katie Bianconi LLC is not financially responsible for veterinary costs related to illness, injury, parasites, accidents, or emergencies
- Katie Bianconi LLC does not guarantee any medical outcome
4. Medication, Feeding & Special Instructions
You'll administer the meds I provide, with the instructions I provide.
If my dog requires medication, I authorize staff to administer owner-provided medication according to my written instructions.
- All medication must be provided in its original container when possible
- Instructions must be clear and in writing
- I am responsible for accurate dosage, timing, and administration instructions
- Katie Bianconi LLC is not responsible for issues caused by incorrect owner-provided instructions, mislabeled medication, missed disclosures, allergic reactions, or side effects
- Katie Bianconi LLC may refuse medication administration if instructions are unclear, unsafe, or beyond reasonable pet care
For food and feeding: I am responsible for providing my dog's food unless otherwise arranged, labeled with my dog's name and feeding instructions. Katie Bianconi LLC is not responsible for illness, upset stomach, allergic reactions, refusal to eat, or other issues related to owner-provided food. If my dog runs out of food, I authorize reasonable replacement food charged to my invoice. All allergies, treat restrictions, and dietary concerns must be disclosed before services begin.
5. Behavior, Aggression & Refusal Rights
Honesty about your dog's behavior keeps everyone safe.
I represent that I have fully disclosed all known behavioral concerns involving my dog, including biting, growling, lunging, snapping, aggression, fear-based reactivity, resource guarding, dog or human aggression, escape attempts, fence climbing/digging/bolting, separation anxiety, destructive behavior, excessive barking, or any history of being declared dangerous.
I represent that my dog has not been declared dangerous or vicious by any government agency, and has no known history of unprovoked attacks causing serious injury, unless fully disclosed in writing prior to services.
I understand failure to disclose behavior concerns may result in immediate termination of services, refusal of future care, additional handling fees, and/or my responsibility for damages, injuries, or expenses.
Katie Bianconi LLC reserves the absolute right to refuse service, separate or isolate dogs, terminate services immediately, require immediate pickup, or deny future participation. This applies to any dog displaying aggressive, dangerous, excessively disruptive, or unsafe behavior — and to any owner who fails to follow policies, make payment, disclose required information, or pick up on time.
If my dog injures another dog, person, or causes property damage, I am solely financially responsible for all resulting damages, medical costs, veterinary expenses, legal fees, and related expenses — including damage to people, animals, property, equipment, fencing, crates, flooring, vehicles, leashes, collars, gates, or furniture.
6. Photo, Video & Social Media Release
Pick your comfort level for photos and videos of your dog.
I grant permission to Katie Bianconi LLC to photograph, video record, and capture images of my dog(s) while participating in services. These images and videos may be used on social media (Facebook, Instagram, TikTok, YouTube), in marketing materials, advertising, website content, promotional videos, printed materials, and educational or informational content.
I waive any right to compensation, ownership, inspection, or approval of such materials and release Katie Bianconi LLC from any claims related to the use of my dog's likeness.
My specific media preference is captured in my pet intake. I understand my dog's name may be used in captions or posts unless I specifically request otherwise in writing.
7. Transportation Consent
If we need to transport your dog, you've okayed that.
I authorize Katie Bianconi LLC to transport my dog(s) as necessary for daycare, boarding, walks, grooming, emergency veterinary care, field trips, pick-up/drop-off services, or related pet care activities. Transportation may occur in personal or company-owned vehicles and reasonable safety precautions will be used.
I accept all risks associated with transportation, including illness, injury, vehicle accidents, sudden stops, escape, delays, or emergencies. I release and hold harmless Katie Bianconi LLC from liability arising from transportation-related incidents except in cases of gross negligence or intentional misconduct.
8. Fees, Schedules & Operating Policies
Standard fees that may be added to your invoice when applicable.

Flea, tick, and heartworm prevention is required. If fleas or ticks are discovered on my dog while in care, I authorize immediate treatment to protect other animals and the facility, with related charges added to my invoice. Katie Bianconi LLC may refuse service to any pet showing signs of flea infestation, parasites, or untreated contagious conditions.
Female dogs in heat: Not permitted to attend daycare, boarding, walking groups, or related services within 28 days from the first observed day of a heat cycle. If a dog enters heat while in care, additional management fees apply and early pickup may be required.
Intact males over 6 months may require additional supervision due to marking, mounting, or dominance behavior. Katie Bianconi LLC reserves the right to refuse participation for intact males displaying disruptive, aggressive, or unsafe behaviors.
Our facility closes at 6:00 PM unless otherwise agreed in writing. Repeated late pickups may result in suspension or termination of services. Current service rates (boarding, daycare, walking, transportation) are posted on puppad.net/pricing and may be updated from time to time.
9. Abandonment Policy
What happens if you don't pick up your dog and we can't reach you.
I agree to pick up my dog on the agreed-upon date and time unless alternate arrangements are approved in writing.
If I fail to retrieve my dog, fail to communicate, or fail to make payment arrangements within five (5) calendar days after the scheduled pickup date/time, my dog shall be deemed legally abandoned to the fullest extent permitted by Florida law.
I expressly authorize Katie Bianconi LLC to take any action deemed appropriate regarding the abandoned dog, including rehoming, adoption placement, transfer to rescue, veterinary surrender, humane placement decisions, retention of ownership rights, or any other lawful disposition.
I remain financially responsible for all unpaid boarding fees, late fees, veterinary costs, transportation expenses, collection costs, and related charges incurred prior to abandonment resolution.
10. Payment & Financial Responsibility
How and when you pay for services.
I am financially responsible for all services, fees, add-ons, extended care, late fees, flea/tick treatment fees, heat cycle fees, intact male fees, boarding fees, daycare fees, walking fees, transportation fees, medication administration fees if applicable, emergency expenses, veterinary costs, collection costs, and any other charges incurred while my dog is under the care of Katie Bianconi LLC.
- Payment is due according to the payment terms provided by Katie Bianconi LLC
- All outstanding balances must be paid before release of the dog unless otherwise approved in writing
- Additional charges may be added to my invoice as described above
- I remain responsible for all charges even if another person drops off, picks up, or communicates on my behalf
- Failure to pay may result in suspension or termination of services
11. Governing Law, Venue & Binding Arbitration
How disputes are handled if anything ever needs to be sorted out.
This Agreement is governed by and interpreted under the laws of the State of Florida.
To the fullest extent permitted by law, any dispute, claim, controversy, or legal action arising out of or related to this Agreement, services provided, unpaid fees, injury, illness, death, property damage, abandonment, or any relationship between me and Katie Bianconi LLC shall first be submitted to binding arbitration, unless prohibited by law or unless the matter is within the jurisdiction of small claims court.
Arbitration shall be conducted in Pasco County, Florida, in accordance with the rules of the American Arbitration Association or similar arbitration service selected by Katie Bianconi LLC. Each party shall bear its own attorney's fees and costs unless otherwise awarded by the arbitrator. The arbitrator's decision shall be final and binding. The parties waive the right to trial by jury except where prohibited by law.
Any arbitration, lawsuit, claim, petition, or legal proceeding arising from or related to this Agreement shall be brought exclusively in a court or legal forum located in Pasco County, Florida. I waive any objection to venue or forum outside Pasco County, Florida, to the fullest extent permitted by law.
12. Electronic Signature Consent
Signing online has the same legal effect as signing on paper.
I agree that this Agreement may be signed by handwritten signature or electronic signature. I agree that electronic initials, checkbox acknowledgments, typed names, digital signatures, scanned signatures, uploaded signatures, or app-based signatures may be used to show consent and intent to sign.
I agree that an electronic copy of this Agreement may be treated as an original. Florida law recognizes that records and signatures may not be denied legal effect solely because they are electronic.
13. Entire Agreement
This document is the whole deal — once signed, it carries forward unless we replace it with a new one.
I have read this Agreement, understand its terms, had the opportunity to ask questions, and agree to be bound by it.
This Agreement remains effective for current and future services unless replaced by a newer written agreement. If any part of this Agreement is found unenforceable, the remaining sections shall remain in effect to the fullest extent permitted by law.
This Agreement supersedes any prior verbal or written agreements related to services provided by Katie Bianconi LLC.
